Reconstrucción quirúrgica con implantes e injertos vertebrales en tuberculosis espinal complicada

La tuberculosis de la columna vertebral requiere cirugía cuando se asocia con déficit neurológico severo, destrucción vertebral importante y compromiso del balance sagital; mediante abordaje combinado con injertos vertebrales y técnicas quirúrgicas dirigidas a lograr la estabilización, el alineamiento y la fusión vertebral. El objetivo del presente trabajo es revisar la evidencia en favor de la eficacia y sostenibilidad en este tipo de estrategias.
Se realizó una revisión sistemática de la literatura entre 2017 y 2023 en PubMed, combinando variables de la enfermedad, del manejo terapéutico y de los resultados; para actualizar los datos y hacer recomendaciones basadas en la evidencia si era posible.
Se incluyeron 11 artículos por cumplir criterios de búsqueda, prefiriendo ensayos controlados aleatorizados, revisiones sistemáticas, metaanálisis, estudios prospectivos y retrospectivos; con un total de 1238 pacientes, todos operados por TBE complicada, con diversos abordajes, técnicas e injertos utilizados, y seguidos durante un rango de tiempo de 9 a 37.5 meses. El análisis de resultados permitió una calidad de la evidencia (GRADE) y una fuerza de recomendación (SORT) que se desarrolla en el trabajo.
La descompresión-estabilización vertebral por vía posterior única o mediante reconstrucción global representan el abordaje más eficaz en el manejo de la tuberculosis torácica y toracolumbar. En comparación con los injertos óseos autólogos, la interposición de jaulas de malla de titanio con instrumentación complementaria es una estrategia de eficacia y eficiencia superior para lograr los objetivos del tratamiento quirúrgico.
